    Motor Freight Return

    Use this form to complete a motor freight return.

    1. Band and wrap the equipment to a pallet so it can be securely transportated.
    2. Submit photos on this form to us.
    3. We will send a truck to come pick up the unit. 
    4. You will receive an email with a Bill of Lading.  Print two copies.  Have driver sign both and then send the signed form to ecommerce@unicosystem.com upon pickup.

    Final Check:

    • Equipment is securely banded to pallet
    • Entire unit wrapped with stretch film
    • All loose parts/accessories secured
    • Pallet is sound, dry, and sized correctly
    • Return label affixed with Order/Invoice
